Advocate for the older population. Health Care Services for Older Adults through HOME VISITS. Older adults & family will be assisted in minimizing health risks through guidance and understanding of: -Personal responsibility for health status to maintain independence and control of health. -Health risk factors that occur with aging which can be modified to prevent complications and decrease risks. -Atypical responses to illnesses and treatment. -Education for compliance with medications and treatments as prescribed. -Improvement of nutrition, hydration, and activities for healthy lifestyle. -Approach to aging with the skill & enthusiasm required.
